To investigate characteristics of cine phase contrast-calculated cerebrospinal fluid (CSF) flow and velocity measures in patients with relapsing-remitting (RR) multiple sclerosis (MS) receiving standard medical treatment who had been diagnosed with chronic cerebrospinal venous insufficiency (CCSVI) and underwent percutaneous transluminal angioplasty (PTA).

[B]MATERIALS AND METHODS[/B]:
This case-controlled, magnetic resonance (MR) imaging-blinded study included 15 patients with RR MS who presented with significant stenoses (≥50% lumen reduction on catheter venography) in the azygous or internal jugular veins. Eight patients underwent PTA in addition to medical therapy immediately following baseline assessments (case group) and seven had delayed PTA after 6 months of medical therapy alone (control group). CSF flow and velocity measures were quantified over 32 phases of the cardiac cycle by a semiautomated method. Outcomes were compared between groups at baseline and at 6 and 12 months of the study by mixed-effect model analysis.

[B]RESULTS[/B]:
At baseline, no significant differences in CSF flow or velocity measures were detected between groups. At month 6, significant improvement in flow (P<.001) and velocity (P = .013) outcomes were detected in the immediate versus the delayed group, and persisted to month 12 (P = .001 and P = .021, respectively). Within-group flow comparisons from baseline to follow-up showed a significant increase in the immediate group (P = .033) but a decrease in the delayed group (P = .024). Altered CSF flow and velocity measures were associated with worsening of clinical and MR outcomes in the delayed group.

[B]CONCLUSIONS[/B]:
PTA in patients with MS with CCSVI increased CSF flow and decreased CSF velocity, which are indicative of improved venous parenchyma drainage.[/QUOTE]

Multiple areas of stenosis and different levels of obstruction of internal jugular and azygous veins (a condition known as cronic cerebrospinal venous insufficiency) recently emerged as an additional theory to the well-known autoimmune concept, explaining etiology of multiple sclerosis (MS). The aim of our study was to evaluate internal jugular vein (IJV) morphology and haemodynamic characteristics in patients with MS and compare it with well-matched healthy individuals and to evaluate the prevalence of venous flow abnormalities in both groups.

Methods
Sixty-four patients with clinically proven MS and 37 healthy individuals were included in our study. In all patients, IJV morphology and haemodynamic characteristics were evaluated by colour Doppler sonography as well as venous flow disorder. The patients were classified into four groups according to MS clinical form presentation. The prevalence of morphological and haemodynamic abnormalities in the IJV were assessed.

Results
The presence of stenosing lesion, mostly intraluminal defects like abnormal IJV valves, were observed in 28 patients (43%) in the MS group, and in 17 patients (45.9%) in the control group (P = NS). By adding haemodynamic Doppler information in the IJV venous outflow was significantly different in 42% of MS patients showing flow abnormalities (27/64), as compared with 8.1% of the controls (3/37), P < 0.001.

Conclusion
In our group of patients, patients suffering from MS had significantly more IJV morphological changes and haemodynamic abnormalities when compared with healthy individuals not suffering from MS. These findings can be well demonstrated by non-invasive and cost-effective Doppler ultrasound.[/QUOTE]

Because of the Buffalo pilot study news release suggesting that venoplasty might actually worsen MS (although the data itself showed no statistically significant changes) the FDA is now requiring evaluations by neurologists, including an EDSS score before and 1 year after venoplasty treatment in the study.

We have decided not to dispute this request because most patients should be able to obtain these evaluations covered by their health insurance. The FDA has also requested that the IR perform ultrasounds to look for blood clots after the venoplasty. We hope this is the last FDA request for revisions. [/QUOTE]

Jolande, ik was gister ergens en Marc was daar ook, ik vroeg hem hoe het tegenwoordig (had hem een paar maanden niet gezien of gesproken) met zijn bloeddruk was (had je om gevraagd). Dat was nog steeds 'perfect' en hij gebruikte nog steeds helemaal geen medicijnen daarvoor. Heb hem trouwens verteld over mijn laatste bevindingen over Robbie en Van Noort. Ook hij vond dat enorm verassend en vroeg aan mij wat de doorslag had gegeven voor mijn gewijzigde indruk. Dat is grappig, zei ik, meneer Aars vroeg dat ook al, maar daarna hoorde ik niets meer van. Hij zei dat hem dat niet echt verbaasde; 'die relnicht wil alleen maar narigheid horen, want zo is ie zelf'. Grappig hè.
