To examine the incidence and distribution of extravascular compression of the extracranial venous pathway (the jugular and/or azygous veins) in multiple sclerosis patients with chronic cerebrospinal venous insufficiency evaluated by mulitislice computer tomographic angiography.

[B]Methods and results[/B]
Study group consisted of 51 consecutive patients with multiple sclerosis in whom chronic cerebrospinal venous insufficiency was diagnosed by color Doppler sonography (CDS). Mulitislice computer tomographic angiography was performed in all patients, and it revealed significant extravascular compression (>70%) of extracranial venous pathway in 26 patients (51%), while in 25 patients (49%) no significant extravascular compression was seen. Extracranial compression due to transverse processus of cervical vertebrae was seen in 23 patients, carotid bulb compression was seen in two patients, and in one case, compression presented as a thoracic outlet syndrome.

[B]Conclusion[/B]
Our data indicate that extravascular compression of the extracranial venous pathway is frequent in multiple sclerosis patients with chronic cerebrospinal venous insufficiency, and that it is mainly due to compression caused by transverse processus of cervical vertebrae. Further studies are needed to evaluate potential clinical implications of this phenomenon.[/QUOTE]

Bij deze meer info over het onderzoek van Roel Beelen;hij mailde mij naar aanleiding van mijn vraag of bosentan een serieuze optie voor mij/ons is.

Het onderzoek wijst er vooral op dat de vasculaire afwijking in de hersenen zelf liggen en dat de hersenen hiervan de oorzaak zijn.

Starten met Bosentan als MS-medicatie is niet simpel. Bosentan is een cardiale medicatie met veel bijwerkingen en moet via een diep veneus infuus gegeven worden. Mogelijk wordt hier wel verder onderzoek naar gedaan onder een geselecteerde groep patiënten.

Omdat het onderzoek aantoont dat de veneuze stenoses een symptoom zijn en geen oorzaak, heeft het weinig zin om deze stenoses te diagnosticeren of te behandelen.

Met deze cytokine-theorie kan ook het 'placebo-effect' worden verklaard: door de ingreep worden de cytokines uit de hersenen weggespoeld. Daarna is er een nieuwe periode nodig om opnieuw tot een schadelijke concentratie in de hersenen te komen.

Het gunstig effect van aspirine kan hier ook mee verklaard worden: aspirine heeft een effect op EGRF, afhankelijk van de patiënt.

Dan zou je toch zeggen dat het wel enig nut heeft om je te laten behandelen, of heb ik dat mis? Ik heb me bijna 3 jaar geleden laten behandelen door Dr. Beelen en tot nu toe shub-vrij gebleven...

Volgens Beelen heeft het weinig/tijdelijk nut, omdat het een gevolg (symptoom) is, geen oorzaak. Een controleafspraak om m'n halsaders te checken vind hij dus ook niet nodig.

Ook ik denk al lange tijd dat ccsvi een symptoom en geen oorzaak is.
Voeding, stress, genetische factoren, trauma, etc. als oorzaken van o.a. ccsvi.

This was a case-control MR blinded 12 months study that included 15 patients with RRMS who presented with significant stenoses (≥50%lumen reduction on catheter venography) in the azygous or internal jugular veins. Eight patients underwent PTA in addition to medical therapy immediately following baseline assessments (cases), while7 had delayed PTA after 6 months of medical therapy alone (controls).CSF flow and velocity measures were quantified over 32 phases of the cardiac cycle using a semi-automated method. Outcomes were compared between the two groups at baseline, 6 and 12 months of the study using mixed effect model analysis.

[B]Results[/B]:
At baseline, no significant differences in CSF flow or velocity measures were detected between the 2 groups. At month 6, significant improvement in both flow and velocity outcomes were detected in the immediate compared to delayed groups, persisting to month 12. Within group flow comparisons from baseline to follow-up showed significant increase in immediate group (p=0.033), but decrease in delayed group (p=0.024). Altered CSF flow and velocity measures were associated with worsening of clinical and MRI outcomes in delayed group.

[B]Conclusions[/B]:
PTA of MS patients with CCSVI increased CSF flow and decreased CSF velocity, which are indicative of improved venous parenchyma drainage.[/QUOTE]

Some multiple sclerosis (MS) patients reported an improvement after percutaneous transluminal angioplasty (PTA) for chronic cerebrospinal venous insufficiency (CCSVI), despite the lack of correspondence with objective outcome scores. The objective was to assess neurologic and quality of life scores before and after PTA for CCSVI in an observational study after a self-decided approach. 44 consecutive MS patients (21/23 M/F; median age 43 years, SD 9.8) who underwent PTA were evaluated before endovascular treatment for CCSVI and after 12 months.

Neurologic outcome was assessed with EDSS, the annualized relapse rate (ARR) and frequency of new lesions at MRI after PTA. Quality of life was evaluated through the MSQoL-54 questionnaire. No modification in the ARR (p = 0.829), worsening of disability status (p = 0.002) and new lesions at MRI in 29.6 % of patients were found, in contrast to an improvement both in physical and mental domains of MSQoL-54 (p = 0.003).

Multiple logistic regression showed EDSS score before PTA to be predictor of an increase of >10 points in MSQoL-54 mental domain (OR 0.52, 95 % CI, 0.31–0.89, p = 0.018). Spontaneously performed approach to CCSVI does not improve clinical and MRI parameters, despite frequent subjective perception of quality of life improvement.[/QUOTE]
